You can call 119 for an NHS COVID Pass (for travel) by post or can use the online form by following link: https://covid-status-letter.service.nhsx.nhs.uk/who-are-you-requesting-for
The NHS COVID Pass letter will be sent to the address you have registered with your GP surgery. It may be the address given when you use other parts of the NHS, like hospitals.
You should get the NHS COVID Pass letter within 7 working days. (subject to change)
