All Nurse and phlebotomy appointments are bookable up to 28 days in advance.
Important: Long-term condition reviews
If we manage one or more of your long-term conditions, we will usually invite you for a routine review once a year.
Where possible, we will send you a booking link during your birth month so you can book any required tests or appointments needed for your review.
This helps us monitor your condition and keep your care safe and up to date.
Appointments can be booked online using services such as the NHS App or Patient Access. If you are unable to book online, please call us on 01803 855897 – option 2.

To help the Travel Nurses assess your travel needs it is important that they are in receipt of the assessment form before your appointment. Please complete the Travel risk assessment on our online platform – Rapid Health at least 12 weeks before travel. Path: Questions and admin (16+) / Travel risk assessment If the form is not returned in time, you may need to arrange vaccinations through a private travel clinic. We recommend checking the National Travel Health Network and Centre (NaTHNaC) website for guidance on non-NHS vaccines and potential travel-related health risks.

Please note that some vaccines require multiple doses and must be given at specific intervals. If you have not heard from us within 10 days of submitting the Travel Vaccination form, please contact the practice.
