We are seeking Care Coordinator’s in Torbay to join a group of dynamic Primary Care Networks. The role will provide a central coordination for patient care planning, including assisting with the booking and delivery of the COVID vaccination programme.
You will be providing a single-point of access for staff & service users, actively managing patients’ care plan delivery or vaccination arrangements. Candidates will need to be flexible, have great communication skills and provide excellent patient care.
As part of the role you will be based in GP practices in Torbay or at the English Riviera Centre in Torquay where the current COVID-19 vaccination delivery is taking place.
This is a 6 month fixed term contract, 37.5 hours per week with a salary of £9-£11 per hour depending on experience. Applicants need to be flexible with working hours which will be between 8am-6pm Monday to Saturday and available for an immediate start.
