You can access this by clicking on the “Contact us online” image on the Home page, or by using the link below. If you are unable to use this option, please call 01803 855 897 and select Option 4. One of our Care Coordinators will complete the form with you. Please note that they will need to ask you several questions to enable them to submit the request on your behalf.

Weekend Appointments (Enhanced Access)
There are clinics available each weekend situated in a central hub location.
The hub location for our patients is:
Pembroke House Surgery, Paignton, TQ3 2EZ.
The appointments offered are by pre-booked appointments only.
