Did you know you can register for the NHS App / Patient Access to enable you to:

From 18th October 2023 you can register for prospective access via NHS App, to access your clinical records including consultations and letters.
The easiest way to get access, is to create an NHS login through the NHS App. When you create an account, you’ll need to prove your identity before you can view your GP health record. This helps keep your record secure.
Proxy Access
Health and care records are confidential so you can only access someone else’s records if you’re authorised to do so.
Children – up until a child’s 11th birthday, the usual position would be for the parents /guardians of the child to control access to their child’s record and online services. On the child’s 11th birthday, GP computer systems will automatically restrict the scope of existing proxy access.
Proxy Access Forms need to be completed by anyone over the age of 11 years old with capacity. If patients lack capacity a representative can complete this on their behalf and state the reason in the ** Other Info.
Adult patients with capacity may give informed consent to proxy access to the practice records about them. People aged 16 or above are assumed to be competent unless there is an indication that they are not. Young people under the age of 16 who are competent may also give consent to proxy access.
Please note you will need to have consent to act on behalf of the patient (or a Lasting Power of Attorney for Health and Welfare) for your request to be considered.

Please read all the information provided, complete the relevant form and return it to us, in person, with your photo ID (i.e. Driving Licence, Passport, Bus Pass, Student Card) or two other legal documents to prove your identity.
