Update your Contact Details – Online Form

When you move house, you need to tell three places in the NHS:

1. Your GP (doctor)
2. Your hospital
3. Your dentist

You can use the form below to tell Compass House Medical Centres your new address, phone number or e-mail address. 

Please remember: we cannot tell your hospital or dentist for you — you will need to contact them yourself.

If you have moved outside of our area (check here: https://compasshousemedical.com/practice-boundary), we might ask you to register with a GP closer to your new home.

If you are updating details for anyone in your family who is 11 years or older, they need to give you permission. We might need to contact them to check.
