Thank you for enquiring about registering with Compass House Medical Centres. We are still accepting new patients at this time. During the Coronavirus pandemic you do not need to present in person to register. You can just print registration forms, complete them and drop them into the practice when we are open. Alternatively you can post your completed forms to us.
Please check our practice boundary – https://compasshousemedical.com/practice-boundary/ to confirm your eligibility to register with us.
All new patients MUST complete the GMS1 Form AND the relevant Adult/ 11+ OR Child under 11 Years Old Registration Form |
GMS1 Form – Family doctor services registration
Adult and Children Over 11 Years Old – New Patient Registration Form
Children under 11 Years Old – New Patient Registration Form
Summary Care Record – Opt Out Form
*Alternatively these forms are available from our Reception Desk
You will be registering with the practice rather than an individual GP. Nevertheless you have the right to express a preference to see any doctor when you contact the surgery to make an appointment. Your medical records may take some time to be transferred from your old practice.
Anyone in England can register with a GP surgery. It’s free to register. You do not need proof of address or immigration status, ID or an NHS number. You might be able to register with a GP surgery that’s not in the area you live. Find out more: https://www.nhs.uk/nhs-services/gps/registering-with-a-gp-outside-your-area/
